Well the simplest way without going to in-depth is to email greg at ICD about it, and what you need to do.
greg_icd@yahoo.com is his E-mail address, so go ahead and askhim about it. As far as i know, it's $125, and turn-around time is like 4 weeks or somethin. Also, if he doesn't mention it, be sure to write "PDS Installation" on the box, or else it will go to the wrong place.
